Our Team

Meet our team who will be responsible for supporting you on your rehabilitation journey.

Jackie Chappel Founder of Positive OTCM

I have worked in the case management industry since 2008 working with both adults and children

Jackie Chappel

Founder & Director

In 2017 I took a brave leap of faith and established Positive OT & Case Management with the desire to focus fully on holistic and practical strategies to enable my clients to reach their chosen goals.

In those days, it was just me and an administration assistant; we’ve grown so much since then!

As well as being the Director of the company, I continue in my role as case manager and occupational therapist. It is important to me that I am able to continue to provide
hands-on support.

My previous roles working for other case management companies, the local authority, and NHS, have helped me gather a wealth of clinical experience which informs my ability to carry out robust assessments and deliver interventions which are meaningful and have a positive impact on quality of life.

I am passionate about making sure therapy is incorporated into everyday life and ensuring you, as the client, are at the heart of everything I do throughout the case management and OT process.

Annette Broadhead

I am an experienced Case Manager who can support you following a traumatic injury, providing specialist services for life changing and life limiting conditions.

Annette Broadhead

Clinical Director

I am an experienced Case Manager who can support you following a traumatic injury, providing specialist services for life changing and life limiting conditions.

I initially qualified as a social worker and my 20 plus years of experience have been gained from working in hospital, hospice, and community settings.

My focus is always on client-centred care, as well as support to your family.  I will work with you to get the right plan in place to support your rehabilitation, making sure all risk assessments are carried out and by commissioning other services to ensure your ongoing needs.

I can also manage your care package if required. I am also familiar with statutory services funding streams and have a sound knowledge of Continuing Healthcare Assessments.

Through my experience in working alongside specialists in rehabilitation including with a prosthetics company, I can provide assessments for amputee clients and provide specialised recommendations for rehabilitation.

Working with amputee clients is of  particular interest to me.

Ashleigh Carnall

I have experience of working with both adults and children, gained over the last 15 years.

Ashleigh Carnall

Lead Support Worker Manager

I have experience of working with both adults and children, gained over the last 15 years.

While working in a respite home as a residential children’s social worker, I supported children and younger adults with a wide range of physical disabilities and challenging behaviours.

I have also worked extensively with young people and their families, in a variety of settings including their homes and the community, as well as
a spinal and brain injury rehabilitation unit.

My key focus would be to support you and your family to make sure you are fully involved in the development of your personalised care plan working alongside the other therapists chosen to work as part of your multi-disciplinary team.

This is particularly important in creating goals in relation to individual rehabilitation needs.

I would also be responsible for supporting the management of your support worker team and assisting with their induction, providing ongoing supervision including supporting their career development and training as required.

It is my role to support the support worker and client to build a positive working relationship which enables our clients to reach their rehabilitation potential in a person-centred way.

Claire Chadwick

The care sector has always held a special place in my heart, and I returned to it when I joined the Positive OT & Case Management team in 2023.

Claire Chadwick

Support Worker Manager

I graduated University with a 2:1 (hons.) degree in Psychology in 2006, and then worked for many years in business support roles for various care companies, specialising in the elderly.

I then made the move to a further education college in 2012 where I worked in Payroll and Human Resources for 11 years.

The care sector has always held the biggest place in my heart though, and it drew me back in. I then came to work for Positive OT & Case Management in 2023.

My role involves working with support worker teams on their induction, ongoing development, supervisions, and general day-to-day management.

I work closely with the case managers, our clients, and their families to create a fully person-centred Support Plan which ensures the team are competent and have all the professional guidance they require to provide the best care and support possible to our clients.

Outside of work, my life revolves around my family. But my other passion is music of all genres, and my favourite place to be, other than home, is at a sweaty concert watching a great band!

Grace Raven

I have worked in a variety of customer-facing roles over the past seven years.

Grace Raven

Joe Hudson

Joe Hudson

Business Services Manager

Joe is short for Joelene. I have never liked my ‘Sunday’ name as when I introduce myself as Joelene, people generally start singing!

I have over 16 years’ experience within the administration side of case management, and I cannot imagine doing anything else.

I really love how case management and occupational therapy can change, enhance, and enrich people’s lives and I am proud to be part of an amazing team that puts clients’ needs first.
