We will be responsible for coordinating the services required to meet your client’s health, care, social, educational and employment needs, and to support their family.
We have a team of fully qualified and regulated professionals operating within the Brain Injury and Complex Case Management (BABICM) Code of Ethics, and who have memberships with the Case Management Society UK (CMSUK) and the Royal College of Occupational Therapists (RCOT).
Our team has extensive knowledge of brain, spinal and multi-trauma injuries as well as amputation. A number of our case managers are advanced practitioner members of BABICM.
Their skills have been developed through many years of experience working in occupational therapy, nursing, social work, management, health and social care roles. We proactively engage with continuous professional development and have access to peer support and supervision.
We understand the importance of building a trusting relationship with your client and their family.
Our team have the essential qualities of diligence, resilience, and compassion. As well as being organised and articulate, our case managers have a flair for being both creative and offer efficient solutions to enhance your client’s life.
Before accepting a new case, we feel it is important to arrange a ‘meet and greet’ appointment to establish compatibility between the case manager and the client and their family.
Our priority is always to focus on a client’s independence and quality of life, with the aim of restoring them to their pre-injury condition.
We will assess and commission the most appropriate services to meet your client’s identified clinical needs.
Working with your client we will agree and implement plans which may involve commissioning a multidisciplinary team, support worker package or increasing client participation in community activities.
Our ongoing support and commitment will include regularly reviewing and reporting on the client’s progress to you, their family, and professionals involved in the case.
