Providing you with a support system
Living with or supporting a family member with a catastrophic life-changing injury can often feel lonely and overwhelming. We are here to help.
Our team of highly skilled case managers will identify the support needed to promote recovery and independence.
We provide guidance and direction to seek the best treatments, care and support using our network of expert therapists and rehabilitation providers.
By creating personalised client centred goal plans, we can monitor and reassess ongoing progress ensuring that desired goals are achieved.
We advocate on your behalf to access the services and care you require.
Whether this is helping you to find or build and maintain your forever home; access innovative technology and therapeutic interventions; arrange a holiday adapted to your requirements or to support with recruiting the best possible care team, we are here to support you.
Throughout the whole case management process, our experienced team will guide you on the different options available and follow through on the choices you make.
We will discuss whether you would benefit with input from specialised therapists from a wide range of backgrounds known as the multi disciplinary team (MDT).
We will assess if you need additional support to get ready each day or if you need assistance to get out and about, or to continue hobbies and activities you have previously enjoyed, and even discover new ones!
Work and education can be a helpful part of your rehabilitation. With support from the MDT we can liaise with your employer, school or college to help your return to your pre-injury roles or even look into new ones.
Your living arrangements are important. We may need to provide options for adapting your existing home to provide access to all areas such as the bathroom, kitchen and bedrooms, and to enable you to get into your garden.
