What we can help with

Throughout the whole case management process, our experienced team will guide you on the different options available and follow through on the choices you make.

We will discuss whether you would benefit with input from specialised therapists from a wide range of backgrounds known as the multi disciplinary team (MDT).

We will assess if you need additional support to get ready each day or if you need assistance to get out and about, or to continue hobbies and activities you have previously enjoyed, and even discover new ones!

Work and education can be a helpful part of your rehabilitation. With support from the MDT we can liaise with your employer, school or college to help your return to your pre-injury roles or even look into new ones.

Your living arrangements are important. We may need to provide options for adapting your existing home to provide access to all areas such as the bathroom, kitchen and bedrooms, and to enable you to get into your garden.

FAQs

Case Management is a collaborative process to assess, plan, coordinate, and monitor services to meet a person’s health needs effectively. It ensures the appropriate care is received while managing costs and resources.

A Case Manager can be essential for individuals with complex medical needs as a result of an accident or injury. The Case Manager assists in coordinating care, accessing resources, managing chronic conditions, and handling insurance or legal matters. A Case Manager can also play a vital role in coordinating medical treatment, documenting care needs, and supporting the litigation process by providing reports and assessments that help the legal team understand the full scope of a client’s injuries and rehabilitation requirements.

Case Managers act as advocates, providing support to navigate healthcare systems, access necessary services, coordinate treatments, and manage insurance or financial concerns. They work closely with a multi-disciplinary team of specialists which can include psychologists, speech and language therapists, physiotherapy, assistive technology and education consultants to ensure the most appropriate care and support is provided throughout the rehabilitation journey.
